چکیده

schemic stroke is the most feared complication of atrial brillation, and many studies have investigated strategies to educe cerebral embolism. Anticoagulation with warfarin is emarkably effective in reducing the risk of ischemic stroke mong patients with chronic atrial fibrillation (1) and has ecome standard therapy. More recently, four randomized rials (2–5) have shown that a strategy of aggressive rhythm ontrol was no better than a strategy of rate control in reventing ischemic stroke and other adverse clinical outomes. Many of the ischemic strokes in patients assigned to he rhythm control strategy in these trials occurred after ormal sinus rhythm had been re-established and anticoaglation had been discontinued.

Role of transesophageal echocardiography-guided cardioversion of patients with atrial fibrillation.

Electrical cardioversion of patients with atrial fibrillation (AF) is frequently performed to relieve symptoms and improve cardiac performance. Patients undergoing cardioversion are treated conventionally with therapeutic anticoagulation for three weeks before and four weeks after cardioversion to decrease the risk of thromboembolism. A transesophageal echocardiography (TEE)-guided strategy has...

Cost-effectiveness of transesophageal echocardiographic-guided cardioversion: a decision analytic model for patients admitted to the hospital with atrial fibrillation.

OBJECTIVES Using a decision-analytic model, we sought to examine the cost-effectiveness of three strategies for cardioversion of patients admitted to the hospital with atrial fibrillation. BACKGROUND Transesophageal echocardiographic (TEE)-guided cardioversion has been proposed as a method for early cardioversion of patients with atrial fibrillation. The cost-effectiveness of this approach, r...

Embolic Events in Patients With Atrial Fibrillation and Effective Anticoagulation: Value of Transesophageal Echocardiography to Guide Direct-Current Cardioversion

OBJECTIVES The primary objective was to evaluate the usefulness of transesophageal echocardiography (TEE)-guided cardioversion to prevent thromboembolic complications in patients with atrial fibrillation (AF) and effective anticoagulation (International Normalized Ratio of 2 or 3) at least three weeks before cardioversion. BACKGROUND Transesophageal echocardiography has been proposed as a metho...

Exclusion of atrial thrombus by transesophageal echocardiography does not preclude embolism after cardioversion of atrial fibrillation. A multicenter study.

BACKGROUND Transesophageal echocardiography (TEE) has been used recently to detect atrial thrombi before cardioversion of atrial arrhythmias. It has been assumed that embolic events after cardioversion result from embolism of preexisting atrial thrombi that are accurately detected by TEE. This study examined the clinical and echocardiographic findings in patients with embolism after cardioversi...

Left atrial appendage-flow velocity predicts cardioversion success in atrial fibrillation.

Restoration of sinus rhythm by electrical cardioversion is a therapeutic option in appropriately selected patients with atrial fibrillation. It is important to determine predictors of electrical cardioversion outcome in patients with atrial fibrillation. Predictive value of clinical and conventional echocardiographic parameters for predicting cardioversion outcome is limited. The role of left a...
